A Consultative Partner for Complex Patients

Prather Medicine brings expertise in geriatrics and palliative medicine to patients and families navigating memory change, complex aging, or serious illness. We join the existing care team as a consultative partner, adding the time and specialized expertise that strengthen the care you already provide.

You remain at the center of your patient’s care. We share what we learn, and we welcome clinician-to-clinician conversations at any point.

    • Diagnostic clarity for memory or cognitive concerns, including diagnosis and staging of mild cognitive impairment and dementia

    • Blood biomarker testing and cognitive testing to diagnose mild cognitive impairment and dementia

    • Comprehensive geriatric assessment for older adults with multiple conditions or medications

    • Medication review and thoughtful deprescribing

    • Symptom management, goals of care, and decision-making in serious illness

    • Palliative care at any stage of illness, alongside ongoing treatment

    • Guidance on prognosis and planning after a hospital stay or new diagnosis

    • Extended family meetings and goals-of-care conversations that deserve unhurried time

    • Patients and families facing complex decisions, functional decline, or caregiver strain

    • Healthy older adults who want a baseline assessment and a plan for aging well

    • Memory & Brain Health Assessment, including personalized cognitive testing and blood biomarkers

    • We use Linus Health to support cognitive testing and offer testing in English, Spanish, German, Hindi, Japanese, Mandarin, and Tamil

    • Comprehensive Geriatric Assessment

    • Palliative Care Consultation at any stage of serious illness

    • Ongoing membership care through Optimized Aging Care, Steady Supportive Care, and Cultivated Care

    • Close support after a hospitalization, a new diagnosis, or another major care transition

    • Visits in our Alexandria office or by telehealth, with home visits for patients with very limited mobility

    • Your patients stay with you. You continue to lead their primary and specialty care, and we add focused geriatric and palliative expertise

    • We order the tests and prescribe the medications we recommend and manage, and we keep you informed of the changes we make

    • Our palliative care works alongside ongoing treatment. When hospice becomes the right choice, we help patients and families understand their options

    • Our physician-led medical care complements the care management and community services your patients may already use

    • We are glad to talk clinician to clinician whenever it would help your patient or you

Referral Process

We welcome referrals from primary care physicians, specialists, and other clinicians, as well as members of the senior care community. Simply share our contact information with the patient or family, and they can reach out directly to explore whether we are a good fit. We are also glad to hear from you when you have questions or information to share about a potential referral.

Prather Medicine is a direct-pay, membership-based practice. We provide a complimentary phone call to ensure patients understand what we are offering and then perform an Initial Assessment. After the Initial Assessment, we provide recommendations and there is no obligation to enroll in an ongoing membership. Their Medicare or insurance continues to cover the care they receive from you. We are welcoming waitlist registrations now, with visits beginning in January 2027.
