Memberships aren’t based on a set number of visits. Every plan includes regular access to your geriatrician and coordination with your family and care team.

Best for: Older adults who are overall well and want a geriatrician in their corner.

Regular supportSteady Supportive Care
Best for: Older adults living at home with early memory changes, several chronic conditions, or who want more regular check-ins.

Visits take place in our Alexandria office or by telehealth. Home visits are reserved for patients with very limited mobility. They are included in Cultivated Care and available in our other memberships for a travel fee. All fees are shared clearly before any commitment.

Advanced illnessCultivated Care
Best for: Older adults living with advanced illness, high symptom burden, frequent hospitalizations, or a major care transition.
